Plan Benefits: There are three levels of coverage Basic, Choice, and Elite. Coverage for in-patient and out-patient medical expenses, Hospital Room & Board, Local Ambulance, Intensive Care Unit, etc. Coverage is per incident with medical expenses ranging from $50,000 to $1,000,000 with a lifetime maximum of $5,000,000. Liaison Student Insurance Cost

The pricing is dependent on the following factors: Premium will depend on the choice of plan- Basic, Choice, or Elite. Along with this choice, the premium is based on the age, the policy maximum, the deductible amount as well as the number of days you need coverage.

Plan Limit: Liaison Student Basic plan is available for ages 14 days to 59 years with per-incident policy maximums of $50,000 through $500,000 and for ages 60 to 64 years old: $50,000 to $250,000 while the Liaison Student Choice and Elite plans are available for ages 14 days to 59 years with per-incident policy maximums of $50,000 through $1,000,000 and for ages 60 to 64 years $50,000 to $250,000. The plans have a lifetime maximum of $5,000,000. All plans have deductible options of $0 (except the Basic plan) $50, $100, and $250

Age: The plan is available for ages 14 days to 64 years

Coverage Days: Premium is based on a daily rate. More the number of coverage days, higher will be the premium. coverage length may vary from 5 days to 364 days and is renewable as long as the primary participant is eligible for the plan.

Liaison Student Insurance – FAQs

Person meeting the following requirements are eligible for coverage as a Primary Participant and
may purchase coverage for their Spouse or Dependent Child(ren):

(i) U.S. Citizens: Students, visiting faculty, scholars, or other persons between the ages of twelve (12)
years old and sixty-four (64) years old with a current passport who are temporarily residing outside the United States and are engaged in Full-Time Educational Activities or Research Activities and have a valid and appropriate visa issued by the Host Country as required. The Eligible Person must remain engaged in Full-Time Educational Activities or Research Activities outside their Home Country during the Period of Coverage.

(ii) Non-U.S. Citizens traveling to the United States: Students, visiting faculty, scholars, or other persons between the ages of twelve (12) years old and sixty-four (64) years old who are temporarily residing outside their Home Country and are engaged in Full-Time Educational Activities or Research Activities or participate in an Optional Practical Training (“OPT”) program in the United States and have a valid J-1, H-3, F-1, M-1 or Q-1 visa or similar appropriate visa. The Eligible Person must remain engaged in Full-Time Educational Activities or Research Activities or engaged in a university-sponsored OPT program outside their Home Country during the Period of Coverage.

(iii) Non-U.S. Citizens traveling outside the United States: Students, visiting faculty, scholars or other persons between the ages of twelve (12) years old and sixty-four (64) years old who are temporarily residing outside their Home Country, are engaged in Full-Time Educational Activities or Research Activities and participating in a qualified educational program outside the United States and have a valid and appropriate visa issued by the Host Country as required. The Eligible person must remain engaged in Full-Time Educational Activities or Research Activities outside their Home Country during the Period of Coverage

Liaison Student Choice and Elite plans cover pre-existing condition through two benefits: Acute Onset of Pre-existing Conditions and Waiver of Pre-existing Conditions. The Basic plan covers them through Acute Onset of Pre-existing Conditions only.

WAIVER OF PRE-EXISTING CONDITIONS

Liaison Student Choice After you have been continuously covered for 364 days, we waive the pre-existing condition exclusion so that pre-existing conditions are covered the same as other conditions subject to applicable copay, deductible, and coinsurance.

Liaison Student Elite After you have been continuously covered for 180 days, we waive the pre-existing condition exclusion so that pre-existing conditions are covered the same as other conditions subject to applicable copay, deductible, and coinsurance.

For US Citizens: Unexpected Recurrence of a Pre-existing Condition covered up to $500.

Sudden and unexpected outbreak or recurrence of a Pre-Existing Condition(s) is one:
(a) That occurs spontaneously and without advanced warning either in the form of Physician recommendations or symptoms and requires urgent care;
(b) That occurs during the initial three hundred sixty-four (364) days of continuous coverage and
(c) For which treatment is obtained within twenty-four (24) hours of the sudden and unexpected
outbreak or recurrence.

A Pre-Existing Condition will not be considered an Acute Onset of a Pre-existing Condition(s):
(i) If the condition is chronic, congenital, or gradually becomes worse over time; or
(ii) If, during the thirty (30) days prior to the acute event, You had a change in prescription or Treatment for a diagnosis related to the underlying Pre-Existing Condition

Your ID cards will have the logo of the network that the plan follows.

In the United States: you can use the United Healthcare Network.

Outside the US: You can go to any provider in the area.  Seven Corners Assist (Toll-free: 800-690-6295)  provides valuable assistance in locating the best possible medical treatment.

The Liaison Studnet plan offers coverage for Medically Necessary Treatment of an Injury resultant of participation in a Non-contact Amateur Sport.

Optional rider is available for Hazardous sports.

The application will need the name, date of birth, visitor’s address, passport country, and email address to send the insurance documents.  If you do have the passport number, you can enter it. A valid credit card will be required to complete the purchase.

Once the application is submitted, an email will be sent to the email address provided on the application which will include links to download the ID card, Visa Letter, and Certificate wording.

Liaison Student Insurance has the following restrictions:

GEOGRAPHIC RESTRICTIONS

State Restrictions – An address in Maryland, Washington, New York, South Dakota, and Colorado cannot be accepted.

Country Restrictions – An address in Cuba, the Islamic Republic of Iran, the Syrian Arab Republic, the United States Virgin Islands, Gambia, Ghana, Nigeria, Sierra Leone, and the Democratic People’s Republic of Korea (North Korea) cannot be accepted.

An address in Alberta and Manitoba, Canada cannot be accepted. If you live in a Canadian province other than Alberta and Manitoba, please call us.

Destination Restrictions –Trips to Antarctica, the Islamic Republic of Iran, the Syrian Arab Republic, Cuba, and the Democratic People’s Republic of Korea (North Korea) cannot be covered.

The Liaison Student Insurance can be bought from 5 days to 364 days and is renewable as long as the primary participant is eligible for the plan.

Before the effective date of the policy:  Full cancellation and refund will only be considered if the written request is received prior to the Effective Date of the coverage.

After the effective date of the policy:  If the written request is received after the Effective Date of coverage, the following conditions apply if the Insured Person wishes to cancel the insurance and a written partial refund request has been made:

a) If any claims have been filed, the Premium is fully earned and is non-refundable.

b) If no claims have been filed with the Company, then a pro-rated refund is possible with a cancellation fee of US $25.

It is a good practice to keep a copy of the entire Claims documents with you before you submit the Claims for processing.
