Claims Process
When seeing a provider (Doctor, Urgent Care, hospital) please print and carry your ID card along with you to the appointment. The providers office will need to see your ID card to reach the Claims Department in order to submit the Claims. If you are hospitalized, please make sure you call the insurance company on the number on the ID card to pre-certify with the stipulated time.
In the event the providers office is out of the network and you have to pay upfront please follow the Claims procedure below
You can download the appropriate Claims form based on the plan you have purchased.
Process to follow for all Claims
- Download and complete the appropriate Claims Form below
- Attach copies of all the pages of the passport
- Attach the itemized bills
- Submit the above within the stipulated time (Most companies have a 90 day limit after seeing the provider)
- Follow up on your Claim by calling the appropriate Claim Number
It is a good practice to keep a copy of the entire Claims documents with you before you submit the Claims for processing.

If you have seen a provider any illness or injury, please download and submit the completed form along with the itemized bills (if you have paid) to the address below. For timely claims settlement, please make sure the claims form is submitted within 90 days of seeing the provider. Please make sure you submit this form even if you have not paid for the expenses. Claims settlement can begin only after this form has been submitted.
